Each plan has its own set of questions. The standards for temporary insurance is much less stringent than for a standard health insurance plan. Nevertheless, these are not guaranteed issue. You will not have to wait around wondering if you are accepted. The simple form has a half a dozen yes/no questions that will tell you immediately if you qualify.

The Fortis and Liberty plans do not have any weight questions. The three year plans do have a height/weight question for underwriting.

1. The medical questions vary but essentially they are interested in knowing if you have had any of the following in the past three to five years (depending on plan).

- hear disorder including heart attack or chest pain
- stroke or circulatory system disorders
- diabetes
- stomach or duodenal ulcer or ulcer symptoms or colitis or Crohn's disease
- uncorrected gall bladder disease
- kidney or liver disease
- alcoholism or alcohol abuse
- chemical dependency or drug abuse
- cancer (excluding basal cell skin cancer)

2. Have you or any person to be insured been hospitalized (except for childbirth) within the past 12 months, due to be so confined or disabled for more than a total of 5 days in the past 12 month?

3. Have you or any person to be insured been declined for insurance due to health reasons in the last 3 years? (This is for health reasons only and does not include for weight or other non health reasons and only in the last three years)

4. Are you, your spouse or any dependent currently pregnant?

5, During the past five years, have you or any person to be insured, tested positive to exposure to the HIV infection, or been diagnosed as having Acquired Immune Deficiency Syndrome (AIDS) or Aids Related Complex (ARC) caused by the HIV infection or other sickness or condition derived by such infection?

If you are not qualified based on these question we might still be able to help you find some coverage. So, if you answered "Yes" to any of these questions you should go to our special guaranteed issue health plan.
